算出来 always involves some kind of calculation in the head. For example, calculating a math problem. 我把这道数学题算出来了. 想出来 can be used for solving any challenge in the head. 我想出来一个解决办法了.

Really good question! You can think of the 个 here as a measure word and it implies you are doing the action for a short amount of time. For example, 吃个饭 means "having a short meal".

Yes you're right! But the 上 in 上大学 and 上课 have slightly different meanings. 上大学 refers more to the general "in college" instead of "starting college" whereas 上课 means "to start class".
