Difference between Nǐ zěnmeyàng? and Nǐ zěnme le?

Although both phrases could mean, "What's up?" or "How are you?"...is there any difference between the two? My guess is that the second is almost implying that something is wrong whereas the first is not making that subtle judgement?

Nǐ zěnmeyàng? (你 怎么样?) 

Nǐ zěnme le? (你 怎么 了?)
