You cannot use 口 to say I have an older brother. You can only use it to describe how many members in your family. Eg. 我家有5口人.
If you wanted to say "I have an older brother" you would say " 我有一个哥哥". :)

Yes, they are not quite the same when talking about intensity or quality of pain. 痛 is used to describe more intense pain... more painful pain!
Other words I suppose you could use to describe pain are 难受. Eg 我的头很难受 or 舒服 Eg. 我的头不舒服:(

疼 is used to describe pain suffered on the body - the degree of pain isn't as intense.
我的肚子很疼 or 她今天有点头疼。
痛 -Can also be used to describe pain suffered on the body eg. 她的脚痛。However can also be used to describe more intense pain. Eg. when my boyfriend broke up with me 我的心痛. It didn't 't just "hurt" it " broke" in this case you wouldn't use 疼. It can also be used to describe pain with diseases and ailments.

Apologies for this, there is an error in the English translation.... it should be "He is also rude and impolite". In this case the 还 means "also". The guy is agreeing to the previous statement that the man is 很自私,也很懒惰, and adds that he is also 还很无礼.
