Thank you very much for the difficult question for bright shiny Sunday morning. I guess that might be a typical question even natives and teachers are not so happy to answer, hehe. I also look forward to hearning answers from Chinesepod teachers.

All I can say is that "他来过" strongly connotes "an action in the past" and "he is not here anymore". The "的时候" is actually superfluous, but acceptable. By the same token, you can say "十二月份的时候", for example. As for "他来了", let's wait and see, hehe.

You're right. That's an exception. "Officially" you can't say "两两" because the two consecutive "liang" sounds a little clumsy. In general, you can use both 二 and 两 for weight and measures such as 斤,尺,吨,and 公里.

In that case, you should use "二" (er4), but not 两(liang3). The usage of "两" (liang3) is a little complicated, but basically you use 两 when it's used in combination with a measure word, such as 两个人 (two person) and 两台电脑 (two PCs). In the case a number is double-digit (and more), you have to use 二 (er4), just like 十二个人 (twelve persons).

about twelve times = 大约十二次

Both "请再说一遍" and "请再说一次" are acceptable, and probably the former one is more commonly used in China. Theoretically "说一遍" connotes a little more "repeat carefully" than "说一次" does, since "遍" indicates "cover to cover".
