June 17, 2013 at 2:22 AM出问题 is more like a problem has arisen or appears for some reason or another in a situation (a problem has come up or shown up out of no where) and 有问题, is like someone or something has a problem so after a problem arises (出问题）something or someone could have a problem （有问题）.
