I'm pretty sure addressing someone as 陈同志 ie putting their surname first, can only be interpreted as Comrade Chen.

Some similar ones: 老大妈,老奶奶,老兄,老弟,丫头 etc.

Is my understanding correct that if  老头 (address to old man) is disrespectful if said 老头儿 but a wife can call her husband 老头子 and that's OK (well at least not any more disrespectful than usual ;-)

I have tried it (and sometimes accidentally wore the wrong set of shoes and only realized later). I am sceptical of these theories too for two reasons. My running style is highly "unnatural" -- run 30-60 minutes every 2-3 days on flat and hard surfaces. A hunter-gatherer would presumably run hours over varied terrain and then rest days. Sadly, my lifestyle cannot accommodate this pattern.

Furthermore, ideas coming from civilizations or periods where the life expectancy is low (the high expectancy of the Mexican Indian tribe you mentioned could be explained by genetic differences) really do need to be taken with a pinch of salt by someone like me on the other side of 40, and hoping to continue until 80 (a link for you: http://www.nytimes.com/2010/11/28/magazine/28athletes-t.html)

Not sure how that book you mentioned ended up on my Kindle -- blame one-click ordering I guess.

If you're 1.60 and weigh 60 kilos you can do this. I would not survive even a 2km run. Also Chinese people sweat less, so a wicking fabric is not really needed.

If that makes me sound like I'm tall, fat and sweaty, not far off the mark, especially according to Chinese perceptions.

Thanks Connie! Google agrees that 一匹狼 is more frequent than the others.

But, 一匹大灰狼 is pretty rare. My theory is that ”大灰狼“ is mostly seen in children's stories and using 只 is used to 可爱化 a bit.
